At Van Otis, we are appreciative of the support we have received since 1935! We give generously to many local causes, but we are unable to fulfill all requests. We also make every effort to ensure that our requests rotate among charities each year.

Through our support of charities, we desire to support the friends and families of our longtime customers and associates. In order to be considered, please ensure that:

• Your form is submitted via the form below rather than phone, fax, or email
• Your cause is beneficial to the health, well-being, or education of people in need
• Your request is 4 or more weeks prior to the event
• Your organization is a tax-exempt nonprofit
• The form is completed

It is not necessary to follow-up with a phone call, as an e-mail response will be sent within one week of receiving the request.
